Yes. A tax office income certificate (Form A) is required from every adult household member. Those settling with a lump-sum tax submit a certificate from the head of the tax office (Form B). Additionally, every person submitting Form A must attach a ZUS or KRUS certificate of health insurance contributions in the base year.
A KRUS certificate is required in three cases: when the family receives income paid by KRUS (pensions, annuities), when someone, while owning a farm, also earns other income subject to PIT taxation, or when someone runs special sections of agricultural production. Contributions paid by that person are deducted from the income.

For the social scholarship, the scholarship for students with disabilities, and emergency aid, you will receive a call to supplement missing documents within a set deadline. If you do not complete the documents, the application remains unresolved. For the rector’s scholarship there is no supplementation: an undocumented achievement is not scored.
